If you require a re-issue of your degree or VET Testamur because you have lost your original document:
- If you have lost your original Testamur/Certificate, you can complete the “Request for Academic Documentation’ form located on the website here.
- Complete a Statutory Declaration form stating that you have lost your original document and require a new copy. The Statutory Declaration form includes a list of those persons able to sign and certify your declaration form (attached).
- Complete both forms and submit together to student.records@endeavour.edu.au
- We will be back in touch to confirm the re-issue fee and how to pay.
If you require a re-issue of your degree or VET Testamur because your original document is damaged:
- If you have damaged your original Testamur/Certificate and would like to order a new copy, you can complete the “Request for Academic Documentation’ form located on the website here
- You must surrender your original damaged Testamur/Certificate by posting it to your nearest Campus or directly to the Office of Student Records located at the Brisbane Campus. Addresses for each Campus can be found on the website here.
- Complete the Request form and submit to student.records@endeavour.edu.au advising where you have posted back your original document.
- We will be back in touch to confirm the re-issue fee and how to pay.
